 Candidates should deposit the relevant fee through bank challan from "fms.uhs.edu.pk" Link 1 or any branch of Bank of Punjab (BOP)

 can and attach the original receipt of the challan with the application.

 BPS 1 to 4 : Rs.100

 BPS 5 11: 500 Rs

 BPS 17 : 2000 Rs

 Printout of completed application form, attested education certificate, experience certificate, national identity card, domicile, passport size photographs and bank
